En effet, il n'y as pas d'array, par contre j'ai trouver ça dans la docs, j'ai tester et ça marche impec :

// Descibe checkbox Field
$m->add_checkbox(Name, Text, Value, Obligatoire, Checked);
Name => the name off the field => Nom du champ
Text => the comment value for the field => Intitulé du chap
Value => the value if the checkbox is « checked » => Valeur si Coché
Obligatoire => False (allways) => Obligatoirement à False
Checked => the default value (ture or false) => Coché ou non par défaut
